Documents)

A complete copy of the articles of incorporation or organization,
including all subsequent amendments, mergers, restatements, and
articles of dissolution, if any. If the file contains restated
articles of incorporation, the record supplied will commence
from the most recent restated articles forward, including any subsequent
amendments, articles of merger, etc., unless you specify "all
charter documents from the beginning".

Same as certificate of status or certified copy, but for use
outside the United States. Certification is prepared
in a form suitable for application of an Apostille or Authentication
by the Secretary of State of Wisconsin.

Ordering Use the accompanying
Form SP-48 or provide the information requested by the form. Normal
delivery time varies between 5 to 12 working days depending on the volume
of work in the program. "Next day" EXPEDITED SERVICE is available
for an additional $25.00, per item. (Note: Multiple copies of identical
certificates of status, when ordered at the same time, are subject to
only one Expedited Service fee.)
Delivery Materials are sent via US Mail, unless you specify delivery
by your own courier service. Provide the name of your carrier and your
account number. Upon request, a Certificate of Status (but not
certified copywork) may be transmitted to a FAX number you provide.
Only the FAX item will be sent, unless you indicate that you also want
to purchase a hardcopy of the certificate.
Payment If you order by mail, remit the service fee by check (payable
to Department of Financial Institutions) or provide your Visa or MasterCard
credit card number, the name as it appears on the card, and its expiration
date.
